There is a preset taken from Moodle 1.9.18 and a preset from Moodle 2.8.6.

The Database activity has been used to create a Support Log for entering a record of support phone calls, tracking the status of each support ticket, duration of calls, assigned staff, problem areas and resolutions.

Modify the field names, add or remove fields or use just as is if it works for you!
